Report an observation
If something doesn't look right after a change has gone live, tell us what you saw. You don't need to work out why it happened — the Project Assurance team reviews every observation.
Why this matters
Frontline staff provide signals. Project Assurance decides what they mean. One serious observation can be enough to trigger assessment — it doesn't need to happen often to matter.
